Submitting Contact Forms

If you received your interview assignment by e-mail from your Area Chair, you were directed to view your applicant’s information in your eBASC Profile. Please submit your contact form using the eBASC 
online system. (You can always find the link to eBASC in the "Quick Links" menu on this website.) Once you click on the applicant’s name, you can view their information and fill out your contact form online.
Fax or Mail
If necessary, you can fax or mail your contact forms to the BASC office using the contact information above. If you are mailing forms, please try to mail them at least a week before the deadline (December 1 for Early Decision and February 15 for Regular Decision) to make sure they arrive in time to be included in the applications. Do not include a cover sheet or any other communication. Receipt will not be acknowledged. Please keep a copy for yourself.
